- Original Machine Readable Passport valid for at least 8 months, with 2 blank pages together.
- Copy of C.N. I. C.

- Medical Vaccination card for meningitis & Polio— for less than 16 years only.
- Affidavit for ladies whose relationship with male “Mehram” cannot be established through Passport name.
